As an Adult Nurse Case Manager, you would be responsible for providing face to face support to our client and their families, whilst creating personalised care plans for each client. You would work closely with a core team of carers training and supporting them with clinical skills and activities.

Key Requirements
  • Registered Nurse with a minimum of 12 months work experience in the UK and an active NMC PIN
  • Tracheostomy and ventilator experience essential
  • ITU/HDU experience would be desirable
  • Care of the spinal patient knowledge desirable
  • Enteral feeding experience essential
  • Excellent written & verbal communication skills
  • Ability to work flexibly and respond quickly to changing priorities whilst meeting deadlines
  • Ability to build strong relationships with clients, their families and carers
  • Supportive management style that can bring out the best in others
  • Confident, passionate and possess a “can-do” attitude 
  • Ability to manage your own diary, your own caseload of community packages which includes working in the community and at Wimbledon head office

Job Summary:
As the Clinical Practice Educator, you would be responsible for providing face to face training to our registered nurses and carers. Whilst supporting and developing the nursing and carer workforce through clinical practice and promotion/implementation of relevant education and development pathways.
Reporting to the Director of Nursing and working in close partnership with the Regional Lead Nurse and Nurse Case Managers the Clinical Practice Educator will promote an educational culture of evidence-based nursing practice.
Key requirements
  • Registered Nurse with a minimum of 12 months work experience in the UK and an active NMC PIN
  • To be a clinical expert in tracheostomy, ventilation, and spinal injury
  • ITU/HDU experience would be desirable
  • Enteral feeding experience essential
  • Excellent written & verbal communication skills
  • Ability to work flexibly and respond quickly to changing priorities, whilst meeting deadlines
  • Supportive management style that can bring out the best in others
  • Confident, passionate and possess a “can-do” attitude
