Job Description

SUMMARY:  Performs medical and health related task designed to ensure the health and safety of people served by NCFL.
 
CLASSIFICATION:                                    Exempt
 
CATEGORY OF EMPLOYMENT:             Full Time
 
HOURS:                                   Minimum 40 hours Base Hours:
                                                         8:30 am to 5:30 pm
 
LOCATION: We are conveniently located in Rochelle Park in Bergen County, New Jersey (near Paramus and Hackensack). We currently operate 12 Group Homes, located in Bergen and Passaic Counties as well as our Achievement Center located in Rochelle Park.

SUPERVISOR:                        Director of Residential Program
 
ESSENTIAL FUNCTIONS: include the following but are not limited to the job specifications contained herein. Additional duties or job functions that can be performed safely may be required as deemed necessary by supervisory personnel. Duties, responsibilities, and activities may change at any time with or without notice.
 

  • Oversees the correct dispensing of medication by checking MAR’s, medications and prescriptions at each residence and at day program.
  • Provide ADL care, transportation, and community outings as needed.
  • Assist with the maintenance of medication books, house log books, etc. to ensure program compliance with all regulations.
  • Ensure medications, medical and safety supplies are ordered and available as needed.
  • Follow admission and discharge requirements for hospitalized individuals.
  • Provide training to both staff and individuals served in relevant areas (i.e. health, hygiene, safety, diet, etc.)
  • Attend staff meetings.
  • Provide coverage at day program or other residential programs as needed.
  • Ensures safety standards and infection control protocols are followed.
  • Applies knowledge of illnesses and injuries in assessing the needs for further medical care.
  • Consults and advocates with other medical professionals and attends IHP’s as needed.
  • Provides efficient patient and care and handles critical situations in a timely and safe manner.
  • Performs and documents monthly assessments of each individual’s vital signs to address health care concerns.
  • Documents and communicates information to other staff and supervisors.
  • Demonstrates good clinical and ethical judgement in the performance of duties and respects the dignity and confidentiality of the individuals we serve.
  • Conduct agency training, including medication and diabetes and seizure, per State requirements.
  • Some travel required between Achievement Center, Administration and Group Homes.
  • Ability to work rotating, on-call schedule.
 
QUALIFICATIONS:

REQUIRED EDUCATION AND EXPERIENCE:
  • Graduate of an accredited School of Nursing with a current State of New Jersey LPN License.
  • BSN college degree preferred.
  • 3-5 years’ experience.
  • Valid driver’s license.
  • Reliable transportation and be insurable and able to pass background screening.
  • Demonstrated ability to assist others in obtaining the knowledge and abilities to monitor and address the needs of the individuals we serve.
  • Should possess or have the ability to obtain the Nurse Practitioner/Nurse Liability Insurance, reimbursable by NCFL.

 
 
WORK ENVIRONMENT: This job operates within the residential homes and day program of NCFL.


PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. Employees are required to hear and talk; they must read and write English. This position is very active and requires the ability to drive, stand, walk, bend, kneel, stoop, crouch, or climb. Employees must often lift over 50 pounds and must have the ability to perform FA, CPR, and Crisis Intervention Techniques.
