Job Description

*RN for Non-Medical Home Care Assessments
*Seeking a compassionate, caring and competent Registered Nurse to perform assessments and develop care plans for our clients receiving non-medical services in their home. This is a part-time opportunity that can expand into a full time position if desired.

Essential Functions:
Performs comprehensive subjective and objective initial assessments and ongoing re-assessment of client status every 90 days.
Formulates an individualized plan of care based on the nursing assessment.
Modifies and updates the plan of care as needed.
Practices nursing with respect for individual, cultural and spiritual differences.
Provides referrals and resources of 4 Seasons Home Care as appropriate which will help our clients thrive and remain safely living at home. Communicates effectively regarding patient's condition.
Evaluation of caregivers
Managing caregiver online educational requirements

Graduate of an accredited school of nursing with one year of clinical experience preferably in home care, long-term care or home health care capacity.
Current Georgia RN license.
Knowledge and ability to provide comprehensive nursing assessments and plan of care for seniors, disabled and handicapped individuals.
Current CPR certification.
Ability to read, write, speak and understand English as needed for the job.
Ability to successfully utilize our web-based system
Possess a valid driver's license and have use of a reliable and insured automobile.
Ability to pass a state and national criminal background check, DMV background check and 3 verifiable professional references.
