Job Description
Home Health Plus is looking for a full time & part time home health Skilled Nurse in GARDEN GROVE, CA. You will be responsible for delivering high quality care to assigned patients.
We do phone interview..
· We offer comprehensive quality of care for long term patients with the home of the patient. All LVNs will be given a detailed plan of care instructed and administered by RN Case Managers. One patient, one location, 9am-6pm shift Monday-Friday, Full time. 11pm-8am shift, Saturday and Sunday, Part time.
· Required care: Patient does have the following: Trach and G-tube. Care consist of monitoring patient status,maintaining activities of daily living, administering medications, suctioning and breathing treatment and feeding via pump. All care is supervised by an RN Case Manager.
Responsibilities:
- Care for ill, injured, or convalescing patients
- Provide basic patient care and treatment
- Collaborate with registered nurses to administer prescribed medications
- Educate and update patients on medical treatments
Qualifications:
- Previous experience in home health
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent written and verbal communication skills
