We are hiring an experienced RN for our home health Nurse Manager.  If you are a Texas licensed RN with leadership ability and drive to ensure the quality of patient care in the home, consider joining our growing team. Compensation depends on experience but ranges $80-$85,000/annually.

Duties include:

• Review and develop Plan of Care in accordance with all payors.
• Coordinates services with physician and other personnel of changes in the patient's/client’s condition and needs.
• Supervises teaches and provides clinical direction to other nursing personnel.
• Manage skilled personnel operations.
• Develop short and long-term goals for the department.
• Plan and oversee admission, nursing, and patient care processes.
• Respond to any skilled issues in a timely manner.
• Coordinate with the clinical team to ensure seamless patient care
• Oversee all record-keeping processes and ensure all necessary documents are accurate and up to date.
• Hire, train, evaluate skilled staff.
• Ensure charts are complete for billing and documentation
• Work with CCO for QAPI Program
• Work with Hospice and Pallative services
• Other duties needed to coordinate care and assist the Administrator in leading the Team to provide excellent quality of services

REQUIREMENTS:

• Valid and Current RN License
• OASIS-D training
• Home Health Care policies and procedure knowledge
• Supervisory experience (2 years preferred)
• Extensive knowledge of Medicare and CMS regulations
• Excellent computer skills
• Experience with EMR
• Clinical skills (Wound Care, IV Therapy, Ostomy Care, Foley Care)
• A current Texas driver's license.
• Proof of current and agency accepted CPR.
• Reliable transportation and proof of current and valid auto liability insurance.
• The ability to lift patients when applicable.
• Aptitude for problem-solving.
• Excellent leadership and interpersonal skills.
• Outstanding written and verbal communication skills.

PREFERRED:

• Hospice Experience
• Spanish Speaking
