Position : Registered Nurse in Home Health

TYPE: Full-time 

LOCATION: Muliple locations in the Bay Area

SIGN-ON BONUS: $$$$  1,000  $$$$ after 90-days

Position Summary:

The Neogen Care Home Health RN provides direct patient care under the supervision of a patients physician  Responsibilities include following Start of care plans, providing treatments, Discharges , Wound Care,  Medication administration and working collaboratively with all of the  members of the team to help meet positive patient care outcomes.

Essential Functions:

• Case management & follow-up skilled nursing visits for home health patients.
• Consult & develop treatment plans based on physician's orders and initial patient assessment.
• Hands-on care, management, and evaluation of the care plan
• Report patient care progress to physician and Clinical Manager regularly
• Supervising the work of Home Health Aides / Nurses and provide written care plans
• Discharge patients after consultation with the physician and Clinical Manager,
• Complete clinical / medical documentation on all patients
• Preparing shift reports & update

Required Experience:

• Recent graduate with RN License with at least 2 years of Licensed Vocational Nursing (LVN) will be considered
• Diploma, Associate, or Undergraduate degree in Nursing.
• Current and unrestricted Registered Nurse license.
• Current CPR/BLS certification.
• Minimum 1 year of recent acute care experience in an institutional setting preferred.
• Minimum 1 year of experience in wound care, patient teaching, medication management, lab draws, and IV infusion.
• Experience with Electronic Medical Record Software (Wellsky / Kinnser a plus).
