Our organization is seeking qualified Registered Nurse (RN) for our Medicare Division to include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
