Job Duties

  • Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes.

  • Bilingual (English/Spanish) preferred.

RN / Gerontology Experienced Nurse (private duty / single case / Live-in ) - New England / Florida.

We provide 24x7 coverage with 2-person teams (1 RN & 1RN, LPN or CNA) and are looking for a Registered Nurse, RN to follow the patient to his 3 homes (2 in New England, 1 in Florida, for winters). You would work with and a team of over 10 nurses, aides, and therapists to provide the highest level of patient care in the patient’s homes. Two of the three homes are located in rural areas with distant access to high-quality, immediate, medical care.

Your live-in schedule would include a minimum of three shifts and a max of six, 12-hour, day or night, shifts per week. IN ADDITION (and other than during vacation weeks) you would be required to be on-call and available the rest of each week as back-up to other RN/CNA teams. Guaranteed 1 day off a week, housing, and reasonable travel costs to/from your home each quarter is also included.  Scheduled work weeks run 10 weeks on 3 weeks off, paid travel to your home or destination within the continental US.

Patient: The patient's diagnoses are CHF, NPH, and fall precautions, and cognitive decline. Fall risk.
