Registered Nurse (90036)

POSITION SUMMARY:
Under the supervision of the Clinical Nurse Manager II, the Registered Nurse will provide culturally competent medical, dental and behavioral health services to low-income individuals in Los Angeles County, with a specific focus on the LGBT community.
 
The Registered Nurse will assure quality and cost-effective care for clinic patients, and will manage a patient’s course of treatment, and coordinate care with providers and other clinical support staff to ensure quality patient outcomes are achieved within established time frames with efficient utilization of resources.  In addition, the Registered Nurse will work closely with the Medical Director and Nursing Supervisor to design, implement and monitor workflow processes throughout all APLA Health sites as it pertains to patient centered medical home (PCMH) implementation.  This position is primarily clinical.
                                   
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Provide clinical support to medical providers before and after primary care visits so as to ensure the timely delivery of quality medical care.
  • Interface with medical assistants to ensure patients receive specialty referrals in a timely manner and are able to follow through so as not to interrupt patient care.
  • Treat patients diagnosed with sexually transmitted infections (STIs) in a timely manner and according to APLAHW clinical STI treatment guidelines.
  • Administer vaccines to patients per APLAHW clinical guidelines.
  • Provide and appropriately document health education to patients.
  • Participate in quality improvement activities as directed by the medical director, e.g. data analysis and measurement of outcomes, document and report the results and accomplishments of quality improvement initiatives.
  • Triage patients who walk in to clinic or call with acute medical concerns.
  • Assist front office staff in making appropriate arrangements for patients who arrive late for appointments or for whom an appointment needs to be rescheduled, by reviewing the medical record and determining how soon and where the patient should be seen.
  • Provide guidance for the clinic LVN and MAs by overseeing the front and back office staff with the aim of maintaining efficiency, professional demeanor and courtesy toward patients.  This may include serving as back up for the front and back office when needed.
  • Assist in the clinic transformation to a patient centered medical home (PCMH), in conjunction with the PCMH coordinator, medical director, chief operating office and clinical nurse manager.
  • Maintain adequate medical supplies and medications for clinical areas by monitoring supplies and ordering supplies as needed.  Ensure that exam rooms are fully stocked and prepared for patient care.
  • Participate in chronic disease management for selected patients with special needs.
  • Communicate lab results to patients.
  • Assist patients in linkage to APLA services: benefits counseling, mental health, case management, dental etc.
  • Serve as first filter to eCW patient portal, answering when appropriate within his or her scope.  Forward to appropriate provider when indicated.

REQUIREMENTS:
 
Training and Experience:
A valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing (BRN). Current Basic Cardiac Life Support (BCLS) and Cardiopulmonary Resuscitation (CPR) certification.  Clinical experience as a registered nurse in an ambulatory health care clinic, preferably at a federally qualified health center (FQHC).  Experience working in patient-centered care teams preferred.
 
Must have problem solving and organizational skills in order to ensure a productive work environment and achievement of goals.  Experience in chronic disease management, case management, utilization management and quality improvement projects are preferred. Experience in an HIV medical practice is preferred. 
 
Knowledge of:
Knowledge and experience working with electronic health records, Eclinical works preferred.
 
Ability to:
Ability to provide services in a non-judgmental fashion and work effectively with diverse populations is required as is the ability to maintain records and follow clinical guidelines/protocols.  Must be able to work efficiently and complete tasks with a high degree of accuracy; work and solve problems independently; work flexible hours in order to complete tasks and meet client needs.  Ability to be flexible in handling unanticipated client needs is required.

Must Haves:
-RN license in WA state OR Temp for practice
-1 year of professional nursing experience
-Graduation from an accredited school of professional nursing
 
Job Responsibilities:
  • Directs subordinate nursing personnel on a unit in a hospital or infirmary; serves as a charge nurse of a unit;
  • Assigns staff to patients in the ward; provides counseling and guidance, learning experiences and evaluates performance of staff;
  • Under the direction of a physician, mid-level provider, or supervising nurse, prepares, carries out, evaluates and revises treatment plans for individual patients based on personal observations and on the observations and recommendations of physicians, mid-level provider, psychiatrists, psychologists, social workers, and/or other nurses; coordinates communication between the physician, mid-level provider, social workers, psychologists, and practical nurses concerning nursing care and policies;
  • Assists the physician or mid-level provider with physical examinations, techniques and procedures;
  •  Under the direction of a physician or mid-level provider, treats minor ailments; performs first aid;
  • Holds sick call and examines patients to determine their need for medical care;
  • Records and administers or oversees the administration of medications such as tranquilizers, anti-seizure medications, immunizations, vitamins, blood transfusions, and antibiotics as directed by a physician or mid-level provider;
  • Observes and records the physical and mental condition of patients noting physical and/or behavioral changes, the effects of drugs, and incidents; takes and records blood pressure, pulse rate and body temperature of patients; evaluates physical and mental conditions of patients to determine whether they require the attention of a physician;
  • Takes samples of blood, urine, sputum, feces, gastric contents, gynecological cultures for laboratory tests; instructs patients on giving sputum, urine, and other specimens;
  • Cares for and counsels ill, injured or infirm patients; provides for the safety, comfort and protection of patients; responds to patients' needs; encourages healthy mental attitudes and new behavior patterns;
  •  In a facility for the developmentally disabled, plans the health services including health screening and examination, development of nursing and residential care plan related to patient health, administration of medicine and other types of therapy within scope of physicians' approval, for a unit; trains residence care staff in handling of severe, multiple handicapped cases and handling occasions which would otherwise become medical emergencies; assures that nursing care programs for each patient are carried out, and reviews and revises these as needed;
  •  Performs and documents for assigned patients: assessment and nursing diagnosis of patients; care planning; implementation of safe nursing care; evaluation of patient responses to nursing care; patient teaching; patient advocacy; admission, discharge and transfer processes;
  • Directs patients, families, visitors and staff members in emergencies and complex situations;
  •  Initiates and evaluates overall care plan; involves patients and their families in care planning;
  •  Applies appropriate policies, procedures, protocols, and standards of practice in performing clinical duties;
  • Participates in quality assurance program by identifying areas for investigation; assists in data collection, monitoring, and the development and implementation of corrective actions for the unit;
  • Contributes to the establishment and implementation of goals for the unit through staff meetings and other activities;
  • Acts as a preceptor for new graduates or experienced employees by assessing learning needs, developing, implementing and monitoring the learning plan, and evaluating the learner's progress;
  •  Acts as a clinical resource to health care team members in identification and resolution of patient care problems to enhance effectiveness of the unit;
  • Sterilizes medical supplies; establishes security measures to prevent loss or theft of drugs and supplies; requisitions supplies; delivers and distributes supplies
