The Registered Nurse is responsible for the supervision and evaluation of assigned caregivers providing personal care services for the medically frail and medically compromised.

Responsibilities include

  • developing a service plan, updates, home supervisory visits, and
    interfacing with caregivers to ensure the service plan is followed.
  • Assisting with on-going maintenance of client files and with on-going administrative tasks as assigned.

Employee’s Duties and Responsibilities:

  • Perform an initial comprehensive physical assessment of a client.
  • Supervise caregivers providing personal care tasks to medically frail or medically compromised clients.
  • Evaluate each assigned caregiver annually, either through direct observation or demonstration of the job tasks the caregiver is required to perform.
  • No supervisor shall knowingly permit an employee who has been exposed to tuberculosis or hepatitis or diagnosed with the same to provide services to clients until it is determined that the employee is not contagious.
  • Complete a Client Service Plan for each assigned client.Complete a Client Service Plan for each assigned client determined to be medically fragile or medically compromised.
  • Make a supervisory home visit to each assigned client’s residence at least every 92 days, starting from the date of initial service. Such supervision shall also include observations
  • Update Client Care plans
  • Make supervisory home visits when Synergy HomeCare of Calhoun receives a complaint and/or incident concerning services and the complaint and/or incident raises serious questions regarding the services being delivered. Document and follow complaint and/or Incident Reporting policies and procedures.
  • Be available to consult with on-call personnel as needed
  • Treat each caregiver and client with respect and dignity.

Preferred Requirements:

  • Microsoft office skills
  • Ability to balance multiple tasks simultaneously.

Qualifications & Education:
  • Graduate of an accredited school of nursing
  • Valid and current RN license in the state of Georgia
  • CPR certified
  • 1-3 years of nursing experience (home care experience, preferred)
  • Supervisory skills, a plus
  • In-home care new client assessment experience preferred.
  • Excellent communication skills, flexible and able to interact positively with the rest of the staff.

  • Coordinate services for patients as needed to other agencies and health care providers as appropriate.
  • Counsel the patient and their family in meeting nursing and related needs.
  • Provide supervision of caregiver services and preparing written instructions for care provided by aides.
  • Maintain confidentiality of patient and agency matters.
  • Submit required documentation in a timely manner.
  • Report observed or suspected abuse pursuant to mandated requirements.
  • Assist the Administrator in decision making and policy formation as it pertains to the Home Care Services.
  • Participate in surveys, studies and special projects as assigned.
  • Maintain accurate time records
  • In addition to scheduled hours, participating in weekend/holiday call schedule, as needed.
  • Observe agency’s attendance requirements and dress code.
  • Conserve agency resources and adequately maintaining agency property, supplies, and equipment.
  • Adhere to all agency polices, including but not limited to, infection control and safety, education, reporting and practice implementation.

Duties include:

  • Review and develop Plan of Care in accordance with all payors.
  • Coordinates services with physician and other personnel of changes in the patient's/client’s condition and needs.
  • Supervises teaches and provides clinical direction to other nursing personnel.
  • Manage skilled personnel operations.
  • Develop short and long-term goals for the department.
  • Plan and oversee admission, nursing, and patient care processes.
  • Respond to any skilled issues in a timely manner.

  • Clinical skills (Wound Care, IV Therapy, Ostomy Care, Foley Care)
