LPR International is seeking an experienced Nurse Practitioner to work full time at the U.S. Army Fort Hunter Liggett Medical Aid Station in Jolon, CA. In this position, the successful candidate will provide medical care and treatment to patients, maintain records and will be responsible for pharmacy operations within the Medical Aid Station at Fort Hunter Liggett, CA to include oversight of the Pharmacy Technician.

 

Qualifications:

Must possess a master's degree in nursing from an accredited educational program in the area of expertise or specialty indicated by the position. Areas of specialties for NP positions include, but are not limited to, Acute Care, Community Health, Family, Geriatric, Home Health or other specialties identified for the position.

 

Education:

  • Graduated from an accredited master's level or doctoral program with additional knowledge to practice in their clinical specialty.
  • Contractor personnel shall be required to meet Continuing Medical Education (CME) requirements in Army Regulation 351-3 (AR 351-3) or as determined by the provider's State of licensure, whichever is more stringent. The Government will not reimburse for these costs.

 

Licensure/Certifications:

  • Possess and maintain a current unrestricted license to practice as a NP in any of the 50 United States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands and be able to obtain clinical privileges to practice medicine at the MTF where services are provided.
  • Must have or obtain a current, valid Drug Enforcement Administration (DEA) registration number prior to performing the contract services. Any change to the DEA requirements to obtain a valid DEA registration number is applicable when the incumbent's existing DEA registration expires.
  • Have and maintain certification as a Nurse Practitioner by the American Nurses Credentialing Centre (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Be certified by a nationally recognized certifying body appropriate to the specialty area of practice.
  • Possess a Bureau of Narcotics and Dangerous Drugs (BNDD) number as required by state agencies where the provider is licensed.
  • Have and maintain current Basic Life Support (BLS) certification. The American Heart Association Basic Life Support for Healthcare Providers is the only program accepted. Advanced Cardiac Life Support (ACLS) Certification is desirable, but not required.

 

Experience:

  • Have a minimum of one year of experience within the last three years working as a Nurse Practitioner.  

 

Specific Duties/Tasks:

  • Provide medical care, to include sick call, and treatment of diseases and/or injuries. Review medical histories and progress of patients. Examine patients, make diagnosis of disease, or injury, and treat patient or prescribe a course of treatment. Abide by all facility rules, regulations, and policies.
  • Seek advice of consultants, when appropriate.
  • Oversee and monitor other medical personnel in the care and treatment of patients to include non­therapeutic services such as physical examinations
  • Set up sterile field and prepares heparinized saline solutions. Maintains aseptic technique, manipulates wires and catheters, anticipates catheter tip location, operates fluoroscopic single-plane and bi-plane x-ray equipment and selects appropriate angulations and positions x-ray table and image intensifier for optimal display of angiographic anatomy while assisting physician in conducting various medical procedures.
  • Prescribe medications, as appropriate, and in accordance with the privileges, hospital rules, and regulations.
  • Prepare documentation of all treatment provided in accordance with hospital directives, ensuring that all documentation and reports are legible and complete. Utilize the appropriate electronic medical record system for patient encounters if available
  • Refer all requests for patient/hospital information that cannot be handled by clinic personnel to the Patient Administration Division. After normal duty hours, requests should be referred to the Administrative Officer of the Day (AOD) or appropriate authorized personnel. Neither the contractor nor HCPs shall release medical information without prior coordination with the AOD.
  • Adhere to the Center for Disease Control guidelines concerning universal precautions and the MTF Infection Control Program at all times while providing services under this contract.
  • Perform all duties in such a manner that they meet all JC standards.
  • Clearly communicate to other health personnel either orally or in writing the findings, diagnosis, and planned treatment regimen or referral.

  • Provide life, limb, and eyesight emergency care as able and immediately transfer patients in a medical or surgical emergency to a hospital under a physician care as necessary.
  • Determine appropriate means of evacuation of emergency patients, prepare patients for evacuation, and arrange for evacuation support.
  • Independently prescribe medications from approved formulary. Administer intra-muscular or intravenous medication as appropriate. Issues temporary profiles according to policy provided by the governing MTF.
  • Communicate in writing and verbally to line commanders/supervisors to recommend appropriate limitations to performance of military duties for patients. The written guidance is provided using appropriate forms such as temporary profiles IA W Army regulations to include placing profiles into eProfile system.
  • Provide clinical supervision of RNs & LPNs and other ancillary personnel who rotate through the Medical Aid Station ensuring patient safety and compliance with MEDCOM regulations on scope of practice for medics and nursing personnel.
  • Be responsible for pharmacy operations within the Medical Aid Station to include oversight of the Pharmacy Technician and reviewing all prescriptions before they are dispensed.

Summary/Objective

The duties and responsibilities of the LPN include managing the day to day healthcare operations of the Community to ensure residents’ healthcare needs are met. The LPN will ensure all residents are treated with respect and dignity as well as managing the quality of their care as their healthcare needs change.

 

Essential Functions

1. Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the responsibilities of a Resident Care Associate in order to meet the residents’ needs and staffing requirements

2. Provides training and education to Resident Care Associates on an ongoing basis to include classroom in-services and situation-specific training; promotes the company’s commitment to compassionate care aligned with our positive approach to care training

3. Assists in the maintenance of resident charts and review of documentation performed by care giving staff

5. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment

6. Ensures and conducts Personal Service Plan reviews and updates, according to Validus Senior Living and state requirements, to ensure residents’ service needs are met

7. Assists in the achievement of occupancy goals by monitoring the health, safety and wellbeing of residents, proactively identifying those at risk for move out

8. Implements timely problem solving

9. Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the Community

10. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Validus Senior Living Guidelines

11. Implements and maintains Validus Senior Living operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences

12. Assists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice both in house and scheduled visits out of the Community. Ensures communication, as appropriate, on the results of these visits

13. Assists in keeping the environment safe for Associates to reduce the occurrence of Workers’ Compensation claims

14. Participates in Community events, including family and social functions

15. Shares on-call responsibilities/shifts as required

16. Other duties as assigned by management

 

Supervisory Skills

The LPN may supervise CNAs, medication technicians and resident care Associates with a focus on compassionate care, resident satisfaction, Associate satisfaction and operational excellence.

Work Environment

Works in a well-lighted area that is ventilated and as physically, physiologically and psychologically innocuous as possible under the conditions which exist at any particular time. On occasion, there may be exposure to outdoor spaces, which will, by their nature, have variances in air quality, temperature, walking surfaces, lighting, and other physical aspects. The noise level is moderate. The noise level is moderate to quiet. Because the essential functions may require exposure to bodily functions and infectious waste, diseases, odors and other situations associated with health care, including but not limited to exposure to AIDS, HIV and Hepatitis B viruses involving Risk Classification Categories 2 and 3**, environmental and safety conditions will fluctuate. To be qualified for the position of LPN, an applicant/employee will need to possess the ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to one’s self or others.

 

Physical Demands

The physical demands described here are representative of those that must be met by an Associate to successfully perform the essential functions of this job. Michal is highly intelligent and educated, and is extremely kind and good-natured. Having four kids so close together in age and being a stay-at-home mom has been very, very difficult for her emotionally and mentally. She was not cut out for being at home, and wants to find work or volunteer opporuntities outside of the home.
