Summary/Objective

Care Partners provide routine care in a manner conducive to the comfort and safety of residents in accordance with federal, state, and local regulations and within the established policies and procedures of the Community.  S(he) is delegated the responsibility in carrying out the assigned duties by the Health & Wellness Director, Executive Director, Charge Nurse or other assigned supervisory personnel to assure that the highest degree of quality compassionate resident care is maintained at all times. Care Partners are customer service experts who interact with leadership, families, vendors, guests, and other associates while displaying exceptional professionalism.

Essential Functions

  1. Help create an atmosphere of compassion, optimism, warmth, resident interest and positive emphasis, as well as a calm environment throughout the Community
  2. Cares for resident per the shift assignment determined by Charge Nurse or Health and Wellness Director.
  3. Responsible for change of shift handoff report to following shift’s Care Partners.
  4. Care Partners who are also certified Med Techs:
    1. May supervise self-administration of medication and treatments as prescribed by the health care provider; during the med pass, the Care Provider observes the resident’s response to the existing treatment plan, is aware of prominent medication interactions and side effects, and observes residents for the occurrence of such and notifies supervisor. The Care Partner, if working as a Med Tech, may communicate with pharmacies and or health care providers as necessary to ensure that residents in the Community have medications as ordered and must document accordingly.
    2. Assist with completion of medication cart audits per policy and direction of supervisor 
  5. Document an accurate accounting of leisure activities, incidents and observations, and any changes in the physical or emotional condition of each resident
  6. Maintain confidentiality and privacy of resident care, procedures, and documentation, and communicate with the resident and families on an adult and mature basis with dignity and respect for the resident’s rights and wishes
  7. Report any resident abuse, accidents or incidents immediately to supervisor
  8. Report changes in the resident’s condition to the charge/staff nurse as soon as possible
  9. Report all complaints and grievances made by residents, families, visiting individuals, agencies, etc. as well as unauthorized trips the resident makes outside the Community to a supervisor
  10. Assure that call system is within easy reach at all times and answered promptly 
  11. Assist with caring for residents’ eyeglasses, hearing aids, prostheses, etc.; Assists in maintaining a physical, social, and psychological environment that is conducive to providing the best interest and welfare of the resident
  12. Bathe, toilet and dress residents; Assist with Activities of Daily Living (ADL)
  13. Follow established safety precautions and observe, monitor and intervene or report unsafe conditions in the
    Community.  I.e. checking and reporting lightbulbs when they need to be replaced, changes in room temperature, etc.

1. Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the responsibilities of a Resident Care Associate in order to meet the residents’ needs and staffing requirements
2. Assists in the supervision of Community-based licensed nursing staff, medication technicians, CNA's, and Resident Care Associates
3. Provides training and education to Resident Care Associates on an ongoing basis to include classroom in-services and situation-specific training; promotes the company’s commitment to compassionate care aligned with our positive approach to care training
4. Assists in the maintenance of resident charts and review of documentation performed by care giving staff
5. As allowed by the Nurse Practice Act, assists in assessing the health, functional, and psycho social status of residents
6. Evaluates the effectiveness and maximizes the resident's opportunity to remain in their environment
7. Ensures and conducts Personal Service Plan reviews and updates, according to Validus Senior Living and state requirements, to ensure residents’ service needs are met
8. Assists in the achievement of occupancy goals by monitoring the health, safety and wellbeing of residents, proactively identifying those at risk for move out
9. Implements timely problem solving
10. Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the Community
11. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Validus Senior Living Guidelines
12. Implements and maintains Validus Senior Living operating standards, systems and processes. Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care Conferences
