Summary/Objective

The duties and responsibilities of the LPN include managing the day to day healthcare operations of the Community to ensure residents’ healthcare needs are met. The LPN will ensure all residents are treated with respect and dignity as well as managing the quality of their care as their healthcare needs change.

 

Essential Functions

1. Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the responsibilities of a Resident Care Associate in order to meet the residents’ needs and staffing requirements

2. Provides training and education to Resident Care Associates on an ongoing basis to include classroom in-services and situation-specific training; promotes the company’s commitment to compassionate care aligned with our positive approach to care training

On occasion, there may be exposure to outdoor spaces, which will, by their nature, have variances in air quality, temperature, walking surfaces, lighting, and other physical aspects. The noise level is moderate. The noise level is moderate to quiet. Because the essential functions may require exposure to bodily functions and infectious waste, diseases, odors and other situations associated with health care, including but not limited to exposure to AIDS, HIV and Hepatitis B viruses involving Risk Classification Categories 2 and 3**, environmental and safety conditions will fluctuate. Summary/Objective
Care Partners provide routine care in a manner conducive to the comfort and safety of residents in accordance with federal, state and local regulations and within the established policies and procedures of the Community. S(he) is delegated the responsibility in carrying out the assigned duties by the Health & Wellness Director, Executive Director, Charge Nurse or other assigned supervisory personnel to assure that the highest degree of quality compassionate resident care is maintained at all times. Care Partners are customer service experts who interact with leadership, families, vendors, guests, and other associates while displaying exceptional professionalism.

Essential Functions
1. Help create an atmosphere of compassion, optimism, warmth, resident interest and positive emphasis, as well as a calm environment throughout the Community
2. Cares for resident per the shift assignment determined by Charge Nurse or Health and Wellness Director.
3. Responsible for change of shift hand off report to following shift’s Care Partners.
4. Care Partners who are also certified Med Techs: a May supervise self administration of medication and treatments as prescribed by the health care provider; during the med pass, the Care Provider observes resident’s response to existing treatment plan, is aware of prominent medication interactions and side effects, and observes residents for occurrence of such and notifies supervisor. The Care Partner, if working as a Med Tech, may communicate with pharmacies and or health care providers as necessary to ensure that residents in the Community have medications as ordered and must document accordingly. Assist with completion of medication cart audits per policy and direction of supervisor
5. Document an accurate accounting of leisure activities, incidents and observations, and any changes in physical or emotional condition of each resident
6. Maintain confidentiality and privacy of resident care, procedures and documentation, and communicate with the resident and families on an adult and mature basis with dignity and respect for the resident’s rights and wishes
7. Report any resident abuse, accidents or incidents immediately to supervisor
8. Report changes in the resident’s condition to the charge/staff nurse as soon as possible
9. Report all complaints and grievances made by residents, families, visiting individuals, agencies, etc. as well as unauthorized trips the resident makes outside the Community to a supervisor
10. Assure that call system is within easy reach at all times and answered promptly
11. Assist with caring for residents’ eyeglasses, hearing aids, prostheses, etc.; Assists in maintaining a physical, social, and psychological environment that is conducive to providing the best interest and welfare of the resident
12. Bathe, toilet and dress residents; Assist with Activities of Daily Living (ADL)
13. Follow established safety precautions and observe, monitor and intervene or report unsafe conditions in the Community. I.e. checking and reporting lightbulbs when they need to be replaced, changes in room temperature, etc.
14. Review Resident Assessment and Plans of Care to residents’ conditions, etc. and explain each procedure to the resident as care is given
15. Follow established universal precautions and isolation procedures and report any communicable or infectious diseases that you may contract
16. Keep resident’s personal articles off of the floor, properly stored, as well as keeping an inventory and assisting to identify resident’s personal possessions on an on-going basis
17. Keep supply room, work areas, and equipment clean
18. Assist residents in preparing for and/or engaging in meals
19. Assist and participate with residents during activities, and social programs

Work Environment

Works in a well-lighted area that is ventilated and as physically, physiologically and psychologically innocuous as possible under the conditions which exist at any particular time. On occasion, there may be exposure to outdoor spaces, which will, by their nature, have variances in air quality, temperature, walking surfaces, lighting, and other physical aspects. Certifications, Licenses, Education and Experience and other Special Requirements

The LPN will have an active first aid/CPR/AED Certification and will maintain CEU requirements throughout employment, as required for licensure. S(he) will preferably have a minimum of two years in the senior living or memory care environment. The LPN should possess exceptional communication skills, both written and verbal as well as strong planning and organizational skills. They should also exhibit exceptional teamwork skills, bedside manner, infection control, health promotion and maintenance as well as the ability to maintain a safe, effective environment. Position Summary:

The Neogen Care Home Health RN provides direct patient care under the supervision of a patients physician  Responsibilities include following Start of care plans, providing treatments, Discharges , Wound Care,  Medication administration and working collaboratively with all of the  members of the team to help meet positive patient care outcomes.

Essential Functions:

  • Case management & follow-up skilled nursing visits for home health patients.
  • Consult & develop treatment plans based on physician's orders and initial patient assessment.

  • Supervising the work of Home Health Aides / Nurses and provide written care plans

Required Experience:

  • Recent graduate with RN License with at least 2 years of Licensed Vocational Nursing (LVN) will be considered
  • Diploma, Associate, or Undergraduate degree in Nursing.
  • Current and unrestricted Registered Nurse license.
  • Current CPR/BLS certification.
  • Minimum 1 year of recent acute care experience in an institutional setting preferred.
  • Minimum 1 year of experience in wound care, patient teaching, medication management, lab draws, and IV infusion.
  • Experience with Electronic Medical Record Software (Wellsky / Kinnser a plus).

Michal is suffering from severe and debilitating depression and anxiety. She has four kids and is unable to care for them right now. She needs someone to be with her during the day to make sure she's not alone and taking healthy actions.

Michal is highly intelligent and educated, and is extremely kind and good-natured. Having four kids so close together in age and being a stay-at-home mom has been very, very difficult for her emotionally and mentally.
