Manages the Company's accreditation renewal process through the CHAP organization within operations, builds strategic goals, and leads a staff for successful on-side visit readiness daily before an accreditation status is renewed for the next three years. Leads direct and monitors the delivery of health care and related health and medical services. Plans develop and implement procedures and policies that enhance the quality of life for people receiving services and assure procedures that support people managing their health care. Care Managers – RNs lead the effort to help prevent costly and traumatic episodes such as avoidable hospitalizations, readmissions, and unexpected kidney failure. For those already on dialysis, Care Managers – RNs provide additional support, particularly around transitions in care such as hospital discharges. required Valid Registered Nurse (RN) certification/license in GA Case Management (CM) (2+ years) preferred 2+ years experience with any of the following: Dialysis Patients Home Health Care Hospice |
