Here is the background. It's important that you read this entire note before contacting me so I don't have to repeat everything on the phone. I'm trying to help my parents with hiring a new aide to take care of my mom. My parents are both 75 and live in Ashby Ponds, an assisted living community in Ashburn, VA. My dad is 100% capable of taking care of himself and drives, plays tennis, prepares some meals, etc. He needs support to take care of my mom, who has advanced Parkinsons Disease, somewhere between stage 4 and 5. My mom uses a walker but frequently gets "frozen" while walking. She has been spending much of her days in bed, only getting up to eat, shower, use the bathroom, and sometimes to go to the Ashby dining room. Daily showering is a must because she has continence problems every day. She also has advanced (but not severe) dementia and her cognitive abilities are extremely limited. Her dementia is related to Parkinsons, but it's not Alzheimers; she still knows her family members' names and how old her grandkids are. However, she's lost concept of time and will forget what year it is or how long ago her mother died (sometimes she forgets her parents are deceased). She needs help with all personal care services. My mom is on several medications, which have to be administered in a very rigid schedule. It is difficult for my mom to have a lengthy conversation, play cards, focus, etc. so when she is in bed, she is watching TV (or just listening to the TV). She does like to do crossword puzzles (someone else reads the clues and writes the answers) and word games. My mom's declining cognition precludes her from having a great quality of life and choices of activities. My parents have had trouble getting consistent help and there is frequent turnover. They need about 50 hours of help per week, about 7-8 hours per day, approximately 2-10 PM. They typically eat dinner in the dining room at Ashby Ponds. No other meal service is provided there. We need someone very patient, engaging, smart, articulate, gentle but physically strong, and able to commit to the full week. A great candidate will also be able to help with laundry and light cleaning while my mom is napping/resting, which is about 2-3 hours per day. Someone who can be encouraging and upbeat and motivate my mom to get out of bed more and walk around (indoors) is ideal. Lastly, it’s important that we find someone whose first language is English. Due to my mom’s cognitive decline, she has a hard time understanding anyone with a strong accent.

The Complex Care Nurse Practitioner works with relevant stakeholders to identify and engage pediatric patients/families in care management with a focus on patient/family experience, improving health and reducing cost. The individual is responsible for working with patients/families to identify strengths and barriers and to develop an individualized, patient-centered care plan. The individual is responsible for direct patient care for patients requiring advanced practice intervention. Excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively are key requirements of the job. ESSENTIAL RESPONSIBILITIES / DUTIES: The complex care nurse practitioner will coordinate with all health care providers serving the child to ensure that they have access to all pertinent information on the child’s medical history, care plans, test results, and scheduled and unscheduled use of health care services. S/he will also work with insurers and local organizations to make sure that children have all needed medical services. The NP will contact the family at least once every six months to monitor the child’s status, review family concerns, and assess the appropriateness of service utilization. S/he will also alert the child’s pediatrician and the Complex Care Pediatrician via secure email to any important new clinical information on the child and also note this information in the EMR and on the care plan. S/he will arrange for case conferences as needed, involving the child’s school or day care and early intervention services providers as appropriate. It will be critical for the NP to oversee transitions in care, particularly from hospital to home. To this end, s/he will maintain close contact with the attending physician and senior resident on the inpatient unit during any periods of hospitalization consult with the Complex Care Pediatrician as appropriate; collaborate with the discharge nurse on post-acute care plans; and ensure a smooth transition from inpatient care back to the child’s pediatrician and the larger community.
Transitions between settings will trigger a review of the care plan by the NP. 1) Care Coordination and Management • Collaborates with other members of the health care team involved in patient care to provide a comprehensive approach to care for both inpatient and outpatients • Educates patients and their families on disease management. • Triages patient questions/concerns, refers to physicians for medical/surgical follow-up as indicated. • During clinical visits completes assessment of current health status, quality of life, side effects and vital signs. • Maintains clinical databases • Serves as liaison to healthcare professionals throughout the hospital.
