Job Description:

The primary purpose of your job position is to plan, organize, develop and direct the overall Wellness operation of the Assisted Living Community and keep it in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be decided upon by you and the Executive Director, to ensure that the highest degree of quality care is maintained at all times.

Daily items to remember:

1. Check with the team on resident care needs and ensure that caregivers are caring properly for our guest. (Morning stand-up with the CMA/CNA/Caregivers to discuss – Alert Charting, Holes in MAR’s, Events that transpired overnight and resident change in conditions)

2. Ensure that all of the assessments and charts are in order. (Functional Capacity Screening form, Service Plan, Fall assessment, Self-Med assessment, etc)

3. MARS/TARS – (Review for holes and verify orders are put in correctly)

4. Alert Charting (If we have a fall, new skin issues or a significant change in condition we should be documenting each day for 3 days on the progress or lack thereof)

5. Communication with Families/Doctors/Pharmacy

6. Review a chart a day (Include TB review, Functional Capacity/Service Plan, Medication review by pharmacy and DNR)

7. Staff Development– The team should see you on the floor supporting the needs of our residents and pitching in wherever possible. Thank your team for the hard work they put in each day, but also be sure and stretch and hold them accountable for all they do.

Job Summary

Under direction of the Physician, the Oncology Nurse Practitioner performs a variety of tasks at St. Croix Cancer Center. In addition to being the trusted patient care partner of the Physician, there is also daily need for coordinating the delivery of oncology treatments, often in intensive care conditions, to assigned patients in a medical oncology facility, ensuring adherence to St. Croix Cancer Center policies, procedures, standards, and practices. The incumbent must work collaboratively with all Clinical services staff in support of direct patient services, exhibiting flexibility and a “can do” attitude. Patient services are the key priority in this position. The incumbent must exemplify the core values and mission of the practice, always exercising utmost discretion, diplomacy and tact in patient/staff interactions.

Essential Functions

  • Mixes and administers chemotherapy.
  • Provides direct care for specified patients, including appropriate supportive care and administration of chemotherapy, blood components, fluid and electrolyte replacements, and other oncology treatments as prescribed.
  • Performs assessments and triage of patient care needs for new and ongoing patients.
  • Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
  • Provides family support as required, including applicable referrals and bereavement counseling.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
  • May train and guide other nursing and support staff engaged in clinical activities; may participate in research and related activities.
  • Performs miscellaneous job-related duties as assigned.

Minimum Qualifications

 

  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of oncology treatment procedures, facilities and equipment.
  • Knowledge of related accreditation and certification requirements.
  • Ability to perform nursing assessments and patient triage.
  • Knowledge of supportive care principles and techniques for oncology patients.
  • Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
  • Ability to maintain emotional stability to cope with human suffering, emergencies, and other stresses.
  • Knowledge of the nature and treatments of oncological diseases and of reactions and side effects of related therapy methods.

  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Associate's degree in Nursing; at least 2 years of experience that can be demonstrated to be applicable to the duties listed in the job description. OR Bachelor's degree in Nursing; US Virgin Islands Licensed Registered Nurse.
  • Prior experience in oncology required.

Registered Nurse

The Registered Nurse is a non- management position, under the direction of Program Administrator, which works with individuals with developmental disabilities within the setting of licensed residential homes

ESSENTIAL DUTIES AND FUNCTIONS: Include the following & other duties may be assigned.

Operational

  • Provide assistance to individuals during emergencies and illness, and notify supervisors based on program procedures.

Health Services

  • Provide general monitoring and assessing of individuals on an on-going basis and as needed.
  • Provide nursing task(s) and monitoring to assigned individuals as prescribed by the primary health care professional.
  • Administer medications following nursing practice and insure that the individuals' rights have been met.
  • Provide tube feedings following standard nursing practice and provide adequate supervision as necessary.
  • Follow proper infection control practices and standards for cleanliness in all activities.
  • Obtain and maintain physician's orders for medications and treatments (both verbal and written), as necessary.
  • Carry the nursing/delegation cell phone as assigned.
  • Monitor medication records, medication supervision records, & medication storage on a random basis and as necessary for nurse delegation.

Delegation

  • Perform assessments on individuals to determine if they are in need of nurse delegation and if so, that they fit the delegation criteria.
  • Following the nurse delegation protocol, perform all aspects of delegation: assess, plan, implement, and evaluate (assessments, write instructions, train the nursing assistants, and supervise the delegation process).
  • Authorize all changes for delegated task(s). Following up with the required paperwork in a timely manner.
  • Tracking the number of preauthorized units each delegated person uses. Then requesting in advance additional units as necessary.
  • Prepare the nurse delegation billing at the 1st of each month.
  • Teach the nurse delegation for nursing assistant classes.
  • Instruct new nursing staff in the healthcare needs of individuals served by SL Start.
  • Provide training to Direct Support Professionals in follow-through techniques prescribed by the health care professional and other care consultants as necessary, and provide regular feedback to the supervisor as to the instruction's effectiveness.
  • Follow established positive behavioral support strategies and programs.

Minimum Requirements

  • Candidates must be at least 21 years of age
  • Possess valid CA driver's license and a good driving record
  • Effective communication skills both written and verbal
  • Demonstrates effective critical thinking and problem solving skills

  • Act according to the guidelines of program and operations manual, program guidelines, company policies & procedures and the employee handbook. Maintain working knowledge of these documents by reviewing them at least once a year.

  • Complete Basic Orientation and attend other in-service training as recommended and/or required.
  • Set and achieve performance and development goals as scheduled through the Annual Employee Assessment and Quarterly Review process.

  • Ensures that nursing care services are non-intrusive and sensitive to the needs of individuals being     supported by SL Start.
  • Provides assessment and medication management for supported individuals.
  • Reviews physician's orders for medications and treatments as necessary.
  • Reads and follows the all individual's Care Plans in order to understand how to best help the individuals that are supported by SL Start.
  • Follows standard nursing practices, proper infection control practices and standards for cleanliness
 
Operational

  • Ensure that delegated tasks promote the individuals retaining or gaining independence.
  • Ensure that delegation training is geared toward staff experience and ability.
