Private outpatient chemical dependence treatment center with two NYS licensed and operating programs - ambulatory detoxification and continuing care therapy is seeking a Registered Nurse to join its team. Services are integrated and include non-opioid medication management, light primary care services, as well as group and individual therapy. The population is primarily high functioning employed patients who are seeking treatment for substance abuse and dual diagnosis issues. This is an innovative medical model program that focuses on cross training between nurses, social workers and physicians.

Position Description: As a Staff Nurse you will be working alongside staff physicians and social workers in the management and treatment of alcohol and/or substance treatment in both the outpatient medically supervised withdrawal and ongoing stabilization programs. A Staff Nurse will be addressing acute disorders associated with alcohol and/or substance use, resulting in determination of level of care and treatment planning for continued recovery. The Staff Nurse participates in the daily medical operations for the programs and collaborates with the Clinical, Medical, and Billing Departments in order to provide safe and effective treatment of chemical dependence and co-occurring disorders. 

Typical schedule Mon-Fri 9am - 6pm or 10am - 7pm. Please note, this position requires rotating weekend coverage about once monthly.

Use clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.

Currently we are awaiting licensure with the state and the DPCS role is a vital position to achieve this goal. Furthermore, DPCS's will be visiting each patient and completing initial assessments per our company policy.

CORE RESPONSIBILITIES:

In accordance with policies and standards established by the Governing Body, and under the direct supervision of the executive team, assists with the planning, coordination, leading, controlling and evaluation of home health services.

DUTIES INCLUDE:

  • Responsible for development and coordination of policies and guidelines for home health services and related administrative systems.
  • Liaison with executive team, Governing Board, and outside agencies and departments.
  • Direct and supervise professional and auxiliary personnel who render client services, perform marketing duties and foster public relations for the organization, consult and plan with health care facilities regarding staffing needs, and communicate the concept of quality client care to the general community.
  • Interpret service needs to the executive team and act as a client services administrative liaison.
  • Participate with the executive team in the formulation of a business plan, i.e., strategic goals, resource allocation plan(s) and a yearly budget.
  • Consult, plan and work with Administrator in policy formulation and programmatic development and implementation including assessing adequate and appropriate staffing.
  • Coordinate with executive team to plan activities and to develop administrative policies and practices for the Agency.
  • Consult with supervisors and staff, individually or in groups, regarding clients or special programs or service programs.
  • Evaluate the program, services, and field personnel.
  • Direct the implementation of service goals and objectives.
  • Develop standards that ensure safe and effective services to clients and families.
  • Establish job functions, qualifications and job descriptions for each service staff (direct and supervisory).
  • Establish and maintain effective channels of communication.
  • Keep program personnel up to date with state-of-the-art clinical information and practices staff development including orientation, in-service education and continuing education.
  • Maintain policy and procedure manuals and updates procedures and job descriptions as policies and procedures change.
  • Direct staff in performance of their duties.
  • Oversee the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.

  • Assist in Agency evaluation activities.
  • Participate in selective conferences and workshops to promote knowledge in community health trends, efficiency in job performance and overall professional growth.
  • Participates] with other health, civic, education and professional groups as directed by the executive team.
  • Serve on and periodically review policies relevant to client care with the Professional Advisory Committee.
  • Assist in marketing the Agency and fostering community relations.
  • Complete initial client assessments in the home on the first day of services provided.
  • Consult with physicians in matters relating to client care services.
  • Evaluate own job performance and utilization of resources in planning for professional growth.
  • Participate in state and national organizations, meetings, seminars, workshops and activities relating to the health professions and health care services, when appropriate.

KEY RESPONSIBILITY: PROMOTE STAFF DEVELOPMENT

  • Utilize capabilities of nursing and paraprofessional personnel in the development of orientation and staff training programs.
  • Recognize leadership potential and offering opportunities for leadership training and development.
  • Encourage and assist personnel to continue self-improvement through formal education, educational meetings and active participation in professional and related organizations.
  • Assist with interviews and hires select home health care personnel.

It is important that the DPCS be able to:

  • Remain available during all hours that services are offered.
  • Demonstrate sufficient background knowledge and expertise in clinical decision-making for the client population of the agency to meet the needs of his or her clients, and to contribute to QAPI processes.

JOB CONDITIONS

  • Position is stressful in terms of meeting deadlines.
  • It requires minimal lifting of office records and printouts.
  • It is primarily a desk job, which essentially involves sitting, standing stooping and walking, as well as an inordinate amount of telephone communication.
  • Travel is required, by car to meet with patients within the Agency’s service area.
  • Must be able to read 12 point and larger type.
  • The ability to communicate well, both verbally and in writing in English is required.

EQUIPMENT OPERATION

The job requires the ability to utilize a PC, calculator, multi-line telephone, and other related office equipment.

COMPANY INFORMATION

Has access to all patient medical records and patient financial accounts, personnel records and company financial records, which may be discussed with all management staff under strict guidelines of confidentiality.

QUALIFICATIONS

  • A Registered Nurse or Nurse Practitioner licensed in CA with a baccalaureate or higher degree in nursing or other health related field with three years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity; or
  • A non - baccalaureate Registered Nurse with four years of experience within the last five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity.
