Required License or Certification:

  • Current California RN license.

  • 1-2 years minimum of recent acute care experience.

  • 2-5 years minimum Home Health nursing experience preferred.

  • Basic computer skills. Knowledge of Word, Excel, and McKesson a plus.

  • Ability to manage time and stay organized. Excellent assessment and nursing skills.

  • Excellent customer service and telephone etiquette skills.

Responsibilities Include:

  • Initiate, develop, implement and revise the patient Plan of Care (POC) in consultation with the patient, physician/practitioner, and other team members to establish patient-specific goals, including therapeutic, preventative and rehabilitative nursing services as appropriate.
  • Perform comprehensive physical assessments and history of current and previous illnesses.
  • Provide services and/or treatments requiring substantial and specialized nursing skill.
  • Coordinate the patient's POC in collaboration with other disciplines.
  • Provide education to patient and/or caregiver/representative regarding treatment and changes to the POC.
  • Complete documentation within the timeframes designated by Medicare requirements and the organization's documentation expectations.
  • Perform supervisory visits of LVNs, Home Health Aides and paraprofessionals providing care to the patient, to include assisting with staff supervisory visits.
  • Participate in team meetings, quality improvement teams, and in-service programs as required.

  2. ensure timely, accurate and complete clinical and support documentation to enable the facility to be fully reimbursed by government payer sources for all services provided, by accomplishing the specific business goals.

KEY JOB FUNCTION:

  • Coordinates the completion of assessments per the OBRA and PPS schedule as appropriate.

  • Timely communicates findings and recommendations to the Director of Nursing and Executive Director.

  • Assist in developing a timely, accurate, and comprehensive care plan for each resident that is individualized with measurable objectives.

  • Completes timely transmission of MDS assessments per Company standards.

  • Timely and accurately corrects rejected MDS transmissions.

  • Analysis and reporting of trends to the QA Committee monthly with suggestions for quality improvement.

  • Supervises additional department staff to ensure completion and accuracy of MDS completion and assigned tasks.

  • Lifts, moves and transports residents, using proper body mechanics or lifting devices for accident prevention.

  • Communicates and interacts effectively and tactfully with the residents, visitors, families, peers and supervisors.

  • Perform all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials as per OSHA guidelines and standards and/or direction of CDC Comply with the System's infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan which reflects OSHA Bloodborne Pathogens Standard to ensure a safe working environment for self and others.

Physical Demands

  • This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling and climbing all day. The employee must frequently lift or move objects and patients weighing over 50 pounds.
