The Nurse is responsible for the Shift coordination and oversight of the nursing process for Residents assigned to his or her care. Responsibilities include both the direct and indirect provision of care according to each Resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice to help the residents to attain or maintain the highest practical mental and psychosocial wellbeing as indicated in their care.

Job Responsibilities:

  1. Integrate values of the Little Sisters of the Poor of Care in all job assignments
  2. Respond to all staff, family members and residents in a courteous, patient and caring manner
  3. Respect the dignity of each person
  4. Communicate in a positive, caring, and helpful manner with residents, family members, and other staff
  5. Knowledgeably and completely perform basic nursing skills according to Federal and State regulations and Company policy using Point Click Care electronic health software
  6. Prepare, administer, and document medications following accepted nursing standards per physician’s orders
  7. Order medications as necessary
  8. Review and revise resident care plans as necessary and as required by regulations
  9. Conduct resident rounds daily, reporting problems to Unit Supervisor and DON
  10. Obtain report from nurse he/she is relieving and record sufficient information to implement appropriate follow-up action as necessary
  11. Provide report to nurse coming on duty, including sufficient information for follow-up action if necessary
  12. Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Charge Nurse
  13. Observe, evaluate and document all changes in the residents’ conditions
  14. Notify physician, when necessary, of changes in the residents’ condition, weight, medication errors, medications not available, abnormal laboratory/X-ray results and other unusual observations
  15. Observe infection control procedures
  16. Identify safety hazards
  17. Provide clinical supervision of nursing assistants providing direct resident care
  18. Perform other related duties as directed


Education/Experience: Graduate of an accredited School of Nursing and currently licensed as a Registered Nurse and or Licensed Practical Nurse in the Commonwealth of Massachusetts.

***Also hiring students who have graduate registered nursing or practical nursing programs approved by the Board or Registration of Nursing the ("Board") or senior nursing year students who are attending the last semester or a board approved registered nurse or practical nursing program to work under and be supervised by a licensed health care provider.***

  1. Performs all job responsibilities in a manner which maintains the resident's rights.
  2. Assesses residents on an ongoing basis and analyses obtained information to develop or revise the plan of care.
  3. Plans, implements and updates individualized care plans for residents based on assessment.
  4. Evaluates the effectiveness of the care plan developed for the resident.
  5. Administers medications to residents, and demonstrates knowledge of drug interactions,
  6. side effects and incompatibilities.  Administers I.V. fluids with appropriate training.
  7. Documents residents progress, deterioration, changes in condition etc. as necessary;
  8. Performs and documents prescribed resident treatments designated to be done by a licensed
  9. nurse.
  10. Abides by both institutional and State Board of Nurse Examiner's requirements in the performing of all duties.
  11. Reports significant changes in residents' condition to the unit manager/supervisor.
  12. Carefully and accurately explains all treatments to resident before performing them.
  13. Assumes charge responsibilities of the unit as assigned.  When in charge:
  • Receives report from the charge nurse on the previous shift and relays information to nursing staff; gives report to incoming charge nurse at the end of the shift; prepares the 24-hour report on computer
  • Accompanies and assists physician on rounds and informs him/her of changes in residents’ conditions’ transcribes, clarifies, and implements physician’s orders.    

Must Have's:

  • Graduate of an approved school of nursing; current Pennsylvania license to practice as a Licensed Practical Nurse
  • Current CPR certification 
  • One year of experience
