PSYCHIATRIC CLINICAL NURSE SPECIALIST ,

Charles River Community Health is an outstanding practice providing medical, pharmacy, dental, behavioral health, and vision services to diverse underserved communities in Allston, Brighton, Waltham & surrounding areas.  The Psychiatric Clinical Nurse Specialist works as integral team member within a dynamic multidisciplinary team to provide direct psycho-diagnostic, psychotherapeutic treatments, psychopharmacological evaluation management to existing clients of the Health Center. Actively collaborates in the formulation of the Health Center’s clinical protocols, policies and procedures. Provides psychiatric consultation, guidance and support to medical, dental and behavioral health providers as well as community health providers when appropriate.  Participates and collaborates on multidisciplinary case conferences for assigned clients.

Responsibilities:
1.Evaluates and treats patients (including walk-ins), provides psychiatric assessment/care, case management, and health education services; prescribes medications as indicated in clinical protocols; assesses psychological impact of illness and treats appropriately; refers patients for evaluation and treatment.
2.Provides consultation to nurses to triage patients who call or walk-in to the Health Center.
3.In conjunction with the Medical Director and Triage Nurses, is responsible for proper storage and dispensing of medications, including monitoring the quality of same.
4.Maintains continuity of care when Health Center patients utilize other health care facilities, i.e. referrals, emergency room services, hospital outpatient departments, specialty clinics and inpatient services.
5.Refers patients for specialty services, in consultation with M.D., provides documentation according to Health Center policies, procedures and follows up on results.
6.Participates in on-site and off-site continuing education programs and ensures training to maintain license and meet the evolving clinical needs of the community and Health Center clients.
7.Works with other staff on planning, development and evaluation of new programs, initiatives and clinical policies.
8.Participates in staff meetings, including quality assurance and medical audit activities, in keeping with Health Center procedures.
9.Participates in clinical staff development activities.
10.Participates in annual performance evaluations for self, as well as providing input in peer review activities.
11.Works collaboratively with other Health Center staff in meeting accreditation and other regulatory and/or funding requirements.
12.Educate patients and family members about mental health and medical conditions, preventive health measures, medications, or treatment plans.
13.Monitor patients’ medication usage and results.
14.Provide routine physical health screenings to detect or monitor health related problems such as heart disease and diabetes.
15.Monitoring of recommended labs.
16.Performs other related duties as required.

HOURS:   Part-Time 16 hours; non-benefits eligible position. This position will involve travel between two locations.

Qualifications:
1.Graduate of Masters of Science in Nursing program
2.Massachusetts Board certification in Psychiatric Clinical Nurse Specialty

TRIAGE NURSE ,

Responsibilities:

1.In conjunction with Nurse Manager and Associate Director of Clinical Operations and Associate Medical Director/Medical Director, works collaboratively with the other clinic services, is responsible for the day-to-day management of on-site clinical and patient related services. 2.Provide and supervise effective triage and floor training to new nurses to the health center and continue to provide clinical support to established nurses.
3.In collaboration with Nurse Manager, responsible for coordinating the development and maintenance of clinical schedules for the RNs to ensure appropriate Provider/Nurse/Medical Assistant ratios and coverage
4.Responsible for receiving sick calls from nursing department and communicating need of coverage between other staff nurses or picking up shifts in conjunction with sister charge nurse and nurse manager when staffing is not available.
5.Functions as a Triage Nurse in the Health Center for 50-60% of the time, and performs management and administrative duties for the balance of the time.
6.Oversight and management of inventory and purchasing of medical supplies for the essential functioning of the medical practice. Supports the maintenance and functioning of the medical equipment to ensure a safe patient and staff environment, and compliance with all applicable regulations and laws.
7.Coordinating preceptorship for nursing students from area colleges and universities

General Summary / Overview
 
The Elliot Lewis Center is looking for an infusion nurse to manage the Center’s infusion suite. We treat approximately 125 infusion patients with multiple sclerosis, many of whom have a monthly infusion. The nurse’s responsibilities include mixing and administering IV medications, monitoring patients throughout the infusions, scheduling patient appointments and organizing their plan of care. The nurse works closely with the MS Center team of doctors, nurses, a social worker, clinical research team and administrative staff to ensure the best possible patient care.  
 
The nurse also deals with reimbursement issues and insurance companies and works with the billing department to guarantee insurance verification and prior authorizations for patient treatment.
 
 Principle Duties and Responsibilities

·         Manage all aspects of the infusion center
·         Perform patient assessment, evaluation and review medications
·         Place peripheral intravenous access; access Mediports and Porta Catheters; draw blood for lab tests when needed
·         Mix and administer IV medications
·         Monitor patients throughout infusions including monitoring vital signs, assessing intravenous sites, adjusting infusion rates, and performing assessments for possible reactions
·         Provide education for new patients and emotional support throughout treatment
·         Document data both on paper and electronically
·         Prepare charts and orders
·         Schedule patients for their treatments and work with the team to organize plan of care (timing of MRI’s, lab work and doctors’ visits)
·         Work closely with insurance companies and the company’s billing department
·         Submit patient insurance information and past medical history for insurance verification and complete prior authorizations as needed
·         Contact specialty pharmacies as needed
·         Order and manage inventory
·         Work closely with the neurologists and other team members
 
    Skills and Abilities Required
  • BSN required
  • Prior IV or infusion experience NECESSARY

The head nurse is:
1)     A vital member of the leadership staff of Camp Wicosuta
2)     Currently an RN license in NH or a compact state
3)     A supervisor of other staff members including general nurses, med nurse, health center assistant
 
Pre-Camp
  • Communicate with health center staff before they arrive at camp to introduce self & answer questions
  • With health center staff, run health center information session during orientation
  • Collect staff medications
  • Review camper/staff medical forms
  • Contact parent as necessary to follow up on concerns and/or questions
  • Place opening health center order
  • Direct the opening setup and on-going maintenance of health center
 
In-Season
 
Health Center
  • Manage supplies and budget (with help of Wico directors)
  • Create on-call & daily schedules for staff
  • Share pertinent info from cabinet meetings with health center staff
  • Maintain on-line camper medical logs with help of staff. 
  • Maintain written health files on every camper & staff member
    • Review previous summer’s health files as needed
    • Store current health files at the end of the season
  • Manage flow of sick-call
  • Manage health check of all staff & campers on arrival days
  • Direct close of camp regarding equipment, activity area, evaluations, etc
 
Staff Supervision
  • Organize & plan health center orientation with help of directors
  • Welcome staff—and orient them to camp (show them their room, give them a tour, make sure they have what they need)
  • With health center staff, plan health center general orientation for all staff
  • Provide daily & as needed feedback to staff in regard to safety, policies, etc.
  • Provide 2 reviews and 1 touch-base to each staff member throughout the summer (4 week staff have 1 touch base & 1 review)
  • Create schedule of regular meetings with health center staff 
  • Attend (with all health center staff) weekly staff meetings
  • With health center staff, provide Wico staff with information about outside health care
 
Additional responsibilities:
  • Take the lead on parent communication
  • Share as much information as possible with directors & head counselors when pertinent
  • Advise camp of any community-wide health concerns
  • Collaborate with Plymouth Pediatrics and other outside providers
  • Make sure at least one nurse is present at all camp activities (if not more) for coverage
  • Attend cabinet meetings at 7:30AM every morning
  • Meet regularly with Corey
  • Assist with opening and closing day of camp – transportation, camper and counselor pick up/drop off and other duties as assigned (check-in, parking lot)
  • Generate and share ideas for camp improvement (staffing, equipment, planning, etc.) with directors.

Assisted Living/Memory Care position: Assist residents with activities of daily living to include: bathing, dressing, grooming, toileting, transfer to and from activities, serve meals etc. Record and report changes in residents' mental or physical condition. Encourage residents to become as independent as possible according to their care plan.
shifts available: 2nd (3pm - 11pm) and 3rd (11pm - 7am)

Qualifications: current CNA license, Flexible schedule, must enjoy working with the senior population.
