Job Responsibilities

Applies nursing judgment through a thorough, systematic approach of applying the nursing process to include:
• Assessment: Conducts comprehensive, timely, and ongoing focused physical, psycho- social and pain assessments; includes assessment of patient education needs.
• Planning: Utilizes the assessment, identifies problems, makes nursing diagnoses, formulates goals, teaching plans and outcomes to create an individualized plan of care. Identifies individual patient goals, readiness to learn and incorporates family into the plan of care.
• Implementation: Prioritizes and implements a competent, prioritized and efficient nursing plan of care.
• Evaluation/Reassessment: Consistently conducts timely evaluation, anticipates and recognizes changes in patient conditions, determines when reassessments are needed and adjusts the plan of care as necessary. Compares expected and achieved outcomes of nursing care utilizing the plan of care.

~~PURPOSE OF POSITION       
Acts as administrative person in charge of Hospital during evening and night shifts, weekend day shifts.  Keeps appropriate Nursing and Hospital Administration informed of important Hospital situations and emergencies.  Functions independently within a broad scope of established policies and goals.          

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
MAJOR ACCOUNTABILITIES         
1. Coordinates activities between hospital departments and acts as liaison between physicians, nurses, families, etc.  Assumes public relations function, responding to press calls, dealing with police, community agencies and other hospitals, as necessary.  Makes arrangements between hospitals for borrowing and loaning equipment.  Assists patients and families with social and behavioral problems.    
2. Serves as an expert clinical resource for patient placement. Works in collaboration with the admitting office to provide appropriate placement of all admissions and transfers from internal and external sources. Acts as central contact with the authority to make all bed management decisions. Monitors bed availability and turnover with input from leadership and staff in all areas. Facilitate continuous patient throughput with effective communication with staff and leadership in all areas.  Facilitates the coordination of patient’s transfer with physicians, staff RN’s, and other departments.      
3. Arranges Patient Services staffing for the following shift, documenting absenteeism.  Also, assesses weekend staffing on a continuous basis, adjusting for changes in patient care, activity or illness/injury of staff members, etc.  Makes hospital rounds to key in on important patient information nursing reports.             
4. Establishes and maintains effective communication with other Managers. Contributes to the evaluation process of nursing staff in conjunction with Nurse Managers.  Provides leadership to staff as necessary. Participates in formal conference sessions with union representatives. Participates in grievance procedure and arbitration, when necessary. Acts as resource person for staff in clinical and administrative matters. Investigates all Unusual Incidents. 
5. Supports, interprets and enforces Patient Services and Hospital Policies and Procedures.  Participates in Patient Services and Hospital committees. Functions as member of code team.  Conducts post code critique.  Promotes quality care through use of the Nursing Process and collaborates with Education/Training Services as needs are identified. Reviews care plans and evaluate implementation based on patient outcomes. Conducts informal patient care conferences.  Assists with patient assessments, as necessary. 
6. Responsibilities involve continual analysis and balancing of priorities to effectively manage all administrative and patient care functions on assigned shift.  Keeps Chief Executive Officer/Chief Nursing Officer informed as appropriate.          
7. Participates in the identification, creation, implementation, and evaluation of Process Improvement projects for assigned areas.
8. Maintains current knowledge of The Joint Commission and other applicable regulatory standards. Participates in regulatory inspections and action plans as needed.  
9. Performs other work related duties as assigned or requested.

JOB DESCRIPTION: 

The RN Is responsible for assessing and reassessing the home care needs­
lndudlng the physical, psychosocial, and environmental needs- of designated dlents and their families. The RN shall be the designated supervisor to all 
care providers performing care for medically fraglle/medlcally compromised dlents as determined by your or designated RN Initial assessment. 

Assessments/reassessments wlll be made by RN to each dlent's place of residence or current location. When designated appropriate, the RN will also supervise all care providers performing PCA and Companion level services by making visits or supervising visits to each dlent's place of residence at least every 92 days (MF/MC dients) and 122 days (companion level dients) or every 90 days for VA dlents (RN only).

These times may change as regulation or client requirements change. Initial assessment visit must be performed by RN. The RN shall document all 
supervisory visits.  The RN shall also conduct assessments of the client's condition, progress and response to services and document these visits In the dlent's service record and scheduled as noted above. The RN develops an appropriate and 
Individual care plan, collaborating with the client's primary caregiver 
(relative or designated careglver/POA) as needed. Acting as the clinical care coordinator for assigned individuals/ famllles, the RN Initiates referrals as 
necessary, supervises and directs PCAs, companions (where designated), 
LPN/RN, families, and other caregivers regarding dlent care. 

GENERAL RESPONSIBILITIES
 
Supervise all PCA providers by making supervisory visits to each client's place of residence at least every 92 days for MF/MC and PCA level clients, and every 122 days for Companion level dlents and provide written documentation of all such visits.

As part of this supervision, the RN Supervisor will never knowingly permit an employee who has been exposed to tuberculosis or hepatitis or diagnosed with the same to provide services to clients until It Is determined that the employee Is not contagious.
Supervise/review competency demonstrations of all PCA providers at least annually and on the job tasks the staff member Is required to perform.
Adhere to agency Infection control and safety procedures by
performing and documenting nursing observations, procedures, and Interventions In the home according to the established care plan In a safe and competent manner.

Effectlvely communicate and Interact with dlents/careglvers and famllles In a supportive manner.
Management of client caseload or other delegated work  responslbllltles In an effective manner within the resources of the agency and according to agency standards.

Interpretation of Agency programs and policies as appropriate to clients and members of the community.
