Scope:  To provide nursing services for basic first aid and basic life support in accordance with the ethical, professional and technical standards set forth by the health care industry. Contracted Nurses will work in our established health care facility and be available to provide health care at localized field trips in and around Sea Girt not to exceed a 5 mile radius

Applicants must have working experience no older than 3 years and possess a proven knowledge of advanced first aid and have an understanding of the physical and emotional health needs of kids ranging from 3rd to 12th grade.

Duties include but are not limited to: communicating health needs of kids to staff and leadership, review of health histories, distribution of family provided prescription and over the counter meds, maintenance of accurate health logs, communicating with parents as needed, able to provide first aid for minor injuries and sickness. 

Primary Functions/Duties: -
  • Provide care to campers and staff in accordance with required treatment Procedures. - Review camper health histories with an emphasis on current illnesses and allergies.
  • Communicating health needs of students to instructors and administration as needed.
  • Administration of doctor prescribed and current prescription and over the counter medication.
  • Providing first aid for minor injuries and illnesses.
  • Referring more serious issues to the appropriate medical personnel.

We have a beautiful waterfront campus in western Massachusetts, with an outstanding group of people where you will have a very rewarding summer. Responsibilities include medical care for 400 children, appropriate paper work for state compliance as well as insurance documentation, transportation to local hospital and medical appointments, as well as dispensing of daily medications. Dates are 6/18/2018 through 8/12/2018, salary is $750-$1000 per week dependent upon experience, plus private room, meal plan, and laundry. The health center is a great place to work and be with children. We have an MD on the premises as well as health center staff of nine. Nurses may wear casual attire. Position Summary:
The Registered Nurse Care Manager’s primary focus is assisting providers and patients in the areas of nursing care management. The RN Care Manager will assist the Nurse Manager as needed and serves as a member of the Care Team.
 
Specific duties and responsibilities:
Work as part of the clinic nursing team to provide a full range of nursing services including floor nursing, patient triage, phone advice, case management, and patient education.

Work closely with the Primary Care Provider and clinical Care Team in providing Nurse Care Management which may include:

-Coordination of care within primary Care Team(s)
-Proactive management of patients with chronic illness with poor clinical outcomes or with complex healthcare needs
-Assist patients in care management and system navigation with recent critical diagnosis
-Assist Providers with management of critical abnormal lab/DI results
-Assist with the care transition for patients discharged from an inpatient setting
-Active participation in Care Team’s “High Risk Case Management” program
-Provide home visits as indicated.

Work with patients and Providers to assess, identify problems, plan goals, monitor and evaluate client  care plans and develop strategies that meet the clients immediate and long term needs.

Work closely with Primary Care Provider to manage patient care needs which could include: medication refills per protocol, lab triage, document management, management of durable medical equipment, prior authorizations of needed medications or other services, and other items as needed.

Assist with population management of patient panels.

Assist patients in obtaining appropriate medications and other medical supplies and equipment.

Participate in patient and staff education.

Provide Nurse visits for designated conditions per clinical protocol/Provider request.

Coordinate Care Team support for patients with new health management goal.

Coordinate tracking of patients for follow-up and continuity of care which may include:
-Developing and using practice guidelines and protocols
-Use of flow sheets, problem lists, and medical data bases
-Development and use of critical pathways.
-Tracking outcomes and reporting to primary care provider/Care Team(s)
-Tracking and expediting referrals.
-Pre-visit planning through proactive daily review of upcoming scheduled                    appointments (gather notes, consults, labs, etc)
-Assist with discharge planning / home care referrals.
-Assist, in collaboration with the Mental Health/Behavioral Health
-May perform IV insertion/monitoring to include fluid rehydration and/or medication administration as directed by the medical provider.
-May perform phlebotomy as directed by the medical provider.
-Completes patient assessment/intake including chief complaint, vital signs, height and weight.
-Acts as resource for Medical Assistants.
-Anticipates the needs of the providers whenever possible.

Ensures that health maintenance needs are reviewed with provider; is aware of age specific screening, GPRA indicators, Diabetes Standards of Care (IHS, ADA)

Carries out health maintenance procedures as allowed by standardized procedures.

Perform electrocardiograms (EKG) as directed by the provider.

Administers medications as ordered by the provider; verifies drug, dosage, allergy status prior to administration.

Ensures lab specimens (those sent to the outside lab) are properly processed including paperwork.

Assists providers with paperwork and collection of records needed to ensure quality patient visits.

Assists with inventory and checks expiration dates for supplies and medications as outlined in Nursing Responsibilities and disposes of expired items via lab or Nursing Director.

Adheres to Blood borne Pathogen Control Plan and disposes of sharps per protocol.

Maintains patient confidentiality at all times; abides by HIPPA and other regulations put forth by the various regulatory agencies.

Performs other duties as assigned.

Qualifications:
 
Graduate of an accredited School of Nursing.  Preferably with a Bachelor=s degree.
 
Current licensure by the California State Board of Nursing as a Registered Nurse.
 
Minimum of five (5) years experience as a RN, including two (2) years of ambulatory outpatient experience.
 
One (1) year of supervisory experience.
 
Possess a current CPR card.
 
HIV pre/post test counseling certification helpful.
 
Preference will be given to Native Americans in accordance with P.L. 93-638.
 
Must be willing to submit to and pass a pre-employment drug screening test.

Education:

Licensure/Certifications:
• Must hold a current license from the Georgia Board of Nursing. Current BLS certification from the American Heart Association (AHA), (Basic Life Support Healthcare Provider card) required. ACLS and PALS from The American Heart Association required upon hire or within one year of hire.
