Home Health RN-Administrator-Clinical manager (Per Diem)

Steady Steps Home Health & Rehab is a new Home Health Care Company, in the process of getting Licensed by Medicare in MA. We serve the Metrowest area with services like Skilled Nursing, PT, OT, SLP, MSW and HHA.

We are looking for a very passionate & experienced RN who wants to enter a start-up and grow with us, and is in for a long haul. This position has a potential to turn part-time/full time. We are new, energetic and looking for a trustworthy employee.

RN-Clinical Manager-Administrator responsibilities include but are not limited to:

• Responsible for review of clinical documentation including OASIS assessments, Coding as per ICD 10 Guidelines

• Day-to-day supervision of clinicians. Documents telephone and in-office conferences with physicians, patients /family, nursing staff, and significant others
• Management/ supervision of payer authorizations

• Assists in taking the referral intakes

• Assists in conducting the monthly clinical record reviews

• Manages the HR needs of the clinical department in collaboration with the Governing Body, notifying the Administrator when new hires are needed to stay proactive with growth plan

• Assists with interviews for select home health care personnel

• Orients professional staff to clinical policies and procedures

• Projects a concerned, professional attitude toward departmental personnel
• Maintains high visibility and availability via telephone while in the office
• Shares on-call responsibility as delegated by Administrator
• Actively develops and pursues referral sources, as well as facilitates on-going relationships with various medical and health communities
• Participates in the monthly PI activities
• Participates in various committees and assumes responsibilities in quality improvement program
• Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders
• Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate
• Administers skilled care visits to clients requiring intermittent professional services and teaches the client, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care
• Qualifications:
• a. current RN license in MA
• b. Posses effective communication skills verbally and in writing, basic computer knowledge and ability to learn/navigate software for EMR
• c. ability to assume a flexible work schedule, and be on call when required
• Requirements:
• a. Must have a current driver's license and auto insurance
• b. Must have at least 2 years of supervisory/administrative experience in Home Health Care

• Provide clinical support to medical providers before and after primary care visits so as to ensure the timely delivery of quality medical care.
• Interface with medical assistants to ensure patients receive specialty referrals in a timely manner and are able to follow through so as not to interrupt patient care.
• Treat patients diagnosed with sexually transmitted infections (STIs) in a timely manner and according to APLAHW clinical STI treatment guidelines.
• Administer vaccines to patients per APLAHW clinical guidelines.
• Provide and appropriately document health education to patients.
• Participate in quality improvement activities as directed by the medical director, e.g. data analysis and measurement of outcomes, document and report the results and accomplishments of quality improvement initiatives.
• Triage patients who walk in to clinic or call with acute medical concerns.
• Assist front office staff in making appropriate arrangements for patients who arrive late for appointments or for whom an appointment needs to be rescheduled, by reviewing the medical record and determining how soon and where the patient should be seen.
• Provide guidance for the clinic LVN and MAs by overseeing the front and back office staff with the aim of maintaining efficiency, professional demeanor and courtesy toward patients.  This may include serving as back up for the front and back office when needed.
• Assist in the clinic transformation to a patient centered medical home (PCMH), in conjunction with the PCMH coordinator, medical director, chief operating office and clinical nurse manager.
• Maintain adequate medical supplies and medications for clinical areas by monitoring supplies and ordering supplies as needed.  Ensure that exam rooms are fully stocked and prepared for patient care.
• Participate in chronic disease management for selected patients with special needs.

• Treat patients diagnosed with sexually transmitted infections (STIs) in a timely manner and according to APLAHW clinical STI treatment guidelines.
• Administer vaccines to adult patients per APLAHW clinical guidelines.
• Provide and appropriately document health education to patients.
• Participate in quality improvement activities as directed by the medical director.
