  • Complete a Client Service Plan for each assigned client.Complete a Client Service Plan for each assigned client determined to be medically fragile or medically compromised.
  • Make a supervisory home visit to each assigned client’s residence at least every 92 days, starting from the date of initial service. Such supervision shall also include observations

  • Make supervisory home visits when Synergy HomeCare of Calhoun receives a complaint and/or incident concerning services and the complaint and/or incident raises serious questions regarding the services being delivered. Document and follow complaint and/or Incident Reporting policies and procedures.

  • Treat each caregiver and client with respect and dignity.

  • Excellent communication skills, flexible and able to interact positively with the rest of the staff.

  • Observe and report symptoms, reaction to treatments, drugs, and changes in the patient’s physical or emotional condition.

  • Coordinate services for patients as needed to other agencies and health care providers as appropriate.

  • Perform medical evaluation of potential donors for automated plasmapheresis procedures
  • Determine donor suitability of new applicants in consultation with a donor center physician
  • Provide limited emergency care, including the administration of medications or treatments
  • Conduct evaluations to ensure compliance with criteria for normal plasma donors
  • Review medical histories and perform suitability examinations
  • Maintain communication logs regarding donor suitability
  • Review normal and abnormal test results to determine continued donor suitability
  • Counsel donors with abnormal test results and defer them accordingly
  • Complete cross-training as required
