Home Care Staff Nurse

The Home Care Nurse Provides all nursing functions according to Massachusetts Law and HHSI policy and procedure and is responsible for medical case management of home-based participants, providing home health services to geriatric clients of the Elder Services Plan while building a trusting relationship with the clients. 
 
Job Duties:
• Performs routine clinical nursing duties, including history taking, vital signs, administration of medications, dressing changes and other nursing procedures under the verbal or written order of a provider as prescribed by law, written clinical policies and within the limits of his/her own license and training.
• Responsible for Case Managing home based clients through communication with providers, center nursing staff, specialists, Clinical Nurse Manager, Clinical Director and all other ESP staff members involved in participant care.
• Completes nursing assessment and collaborates in the development of a comprehensive plan of care, as required by the Patient Care Team.
• Triages patients’ regarding illness and medical therapy and advises the patient/caregiver of the most appropriate action in consultation with a provider whenever necessary.
• Interacts with Clients and family members in a manner that adheres to the highest standards of care and professionalism.
• Builds trust and fosters smooth relationships with Clients and their families.
• Promotes patient confidentiality and right to privacy.  
• Establishes and maintains, via provider order, an ongoing case management system that includes but is not limited to contracting patients with abnormal lab results or who fail appointments in accordance with Agency policies.
• Provides health teaching using appropriate information on health maintenance and disease management during each patient encounter according to Agency policies.
• Continuously updates his/her own clinical and professional knowledge and skills through formal and informal educational experiences.



 
Required/Preferred Education, Experience and Skills:
 
• Current MA RN License and Basic Life Support (BLS) required. 
• Two (2) or more years of experience in a home care, professional or business environment.
• One year of experience working with geriatric patients.
• Must be able to travel in a timely manner to participant's homes in the Brockton and surrounding area. 
• Excellent communication and customer service skills.
• Excellent critical thinking, problem-solving and organizational skills.
• Computer literacy (Microsoft Office applications with a focus on Word, Excel and Outlook).
• Experience with medical computer scheduling preferred.
• Community health or ambulatory care experience preferred.

Nursing Assistant Responsibilities

• As a Nursing Assistant you will help a small group of elders in their daily routines.
• Prepare the elders to get dressed to go out. Escort elders to our vans. The vans drive the elders to our centers, which have a medical clinic and social center.
• The Nursing Assistant will help the elder move from their bed to a chair or a wheelchair. Help them walk, sometimes with a cane or walker.
• Let your supervisor know when the elders need food, medicine, or personal products.
• The Nursing Assistant will follow the care plan to complete tasks for your elders.
• The Nursing Assistant will call the Home Care Nurses when there are changes in an elders' health or emotional state. Call this nurse if there is a change in the elders' needs, or something urgent happens.
• We will schedule your daily assignments. If there is a change to the schedule, the staffing coordinator will call you.

   

  As a Nursing Assistant you must meet these requirements

• A valid Certified Nursing Assistant license
• High School Diploma or GED or 2 years of relevant work experience.
• CPR certificate and First Aid certification. These must be up-to-date.
• You understand basic English, can understand instructions, and write simple sentences.

Under the direction of the Clinic Supervisor, the Clinic Registered Nurse provides nursing services to participants at the clinic by using the nursing process, collaborating with providers and interdisciplinary team members. The Clinic Registered Nurse communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants’ plan of care.

 Duties and Responsibilities:

• Assesses the Participant’s status in person initially, semi-annually and as needed; assists in the development and maintenance of the participant’s plan of care.
• Monitors participant for any acute physical or behavioral changes and adjusts plan of care accordingly.
• Provides skilled nursing care to participants as needed in a timely manner.
• Provides health education, information and counseling to participant, family members and care givers regarding participant’s care needs and safety.
• Implements preventative and rehabilitative nursing procedures as appropriate for the participant’s care and
• Participates in IDT meetings and provides input to IDT, offering clinical expertise in team planning and problem solving.
• Documents in Participant’s medical record and Department records accurately and in a timely manner according to agency standards.
• Provides direction to clinic staff as assigned when the Clinic Supervisor is not present. Offers nursing consultation to other staff members as appropriate.
• Assures quality of care by adhering to therapeutic standards, making or recommending necessary treatment adjustments; following standards of care set by Board of Registered Nursing, Nurse Practice Act, and governing agency regulations. Participates in CEI’s quality assurance performance improvement program (QAPI) as assigned, measuring health outcomes against patient care goals and standards.
• Participates in departmental and agency meetings as directed by the Clinic Supervisor.
• Maintains the confidentiality of all company procedures, results and information about participants or families.
• Complies with all agency training requirements.

  Qualifications:

• Graduate of an accredited school of professional nursing.
• Possess a current California Registered Nurse License.
• Have a minimum of (1) year acute or ambulatory care experience, working with the frail or elderly population.
• Current CPR certification on file.
• Maintains eligibility for licensure by obtaining required continuing education units.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care

1. Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).

2. Provides professional nursing care by utilizing all elements of nursing process.

3. Assesses and evaluates patient’s status by:

4. Writing and initiating plan of care

5. Regularly re-evaluating patient and family/caregiver needs

6. Participating in revising the plan of care as necessary

7. Uses health assessment data to determine nursing diagnosis.

8. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

9. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

10. Counsels the patient and family in meeting nursing and related needs.

11. Provides health care instructions to the patient as appropriate per assessment and plan.

12. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.

13. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

POSITION QUALIFICATIONS

1. Registered nurse with current licensure to practice professional nursing in the state.

2. Graduate of National League for Nursing accredited school of nursing.

3. Maintains a current CPR certification.

4. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

5. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

6. Self directed and able to work with minimal supervision.

7. Management experience not required. Responsible for supervising hospice aides.

8. Demonstrates excellent observation, problem-solving, verbal and written communications; nursing skills per competency checklist.

9. Shows ability to organize and prioritize workload independently.

Healthcare staffing nurse aides perform the following duties and responsibilities:
