Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care; coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members.

•In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness; administers medications in a safe manner consistent with the State of Ohio or Kentucky Nursing Practice Act and Mercy Health policies and procedures; serves as point of contact for patients' care coordination throughout hospital departments.
•Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
•Incorporates the professional practice model into care delivery.
Education:

Graduate of an accredited college/university or institution.

Experience:

Experience preferred.

Knowledge and Abilities:

A.  Good oral and written communication skills.

B.  Demonstrates knowledge and skill in nursing theory and practice of nursing process.

C.  Nurses assigned to a specialty unit will be required to meet criteria established for that unit.

Registration, Certification or Licensure Requirement:

Current Ohio RN licensure; CPR certification required.

ESSENTIAL FUNCTIONS:

The essential functions include:

• Supervise healthcare and protect clients while in their care
• Represent the nursing office at assigned weekly team meetings
• Transcribe orders, receive verbal orders as needed and send order to pharmacy per department protocols
• Administer medications as directed according to agency policy
• Administer first aid as needed
• Review lab results with Nursing Supervisor and Psychiatrist
• Review/prepare and distribute special menus in collaboration with Nursing Supervisor
• Visit and evaluate asthmatic clients on a monthly basis, or more frequently if necessary, and maintain spirometer readings
• Schedule individual and group asthma teaching sessions

• Use appropriate authority to set limits for behavior while clients are in the nurses' office
• Maintain complete, accurate records of all medications administered outlined in agency policy
• Learn and become familiar with Hillsides' pharmacy recordkeeping and legalities involved
• Provide office coverage in the absence of the Nursing Supervisor
• Prepare memos and letters as needed
• Maintain client charts as needed filing pertinent doctor appointments and psychiatric notes
• Transport clients to doctor appointments as required, or for other medical needs and prepare related paperwork in conjunction with Nursing Supervisor
• Weigh and measure clients and maintain monthly records

Qualifications

- Adult patient with a G-tube and seizure disorder. Looking for Saturday and Sunday 12 hour shifts. Tuesdays, 3:30p-9p.

- Adult patient with a G-tube. Looking for Saturday’s 8a-8p.

Lincroft, NJ:

- Ambulatory young adult with a seizure disorder. Looking for Thursday and Friday day shift, 8:30a-4:30p attending school.
