Summary/Objective

Care Partners provide routine care in a manner conducive to the comfort and safety of residents in accordance with federal, state, and local regulations and within the established policies and procedures of the Community.  S(he) is delegated the responsibility in carrying out the assigned duties by the Health & Wellness Director, Executive Director, Charge Nurse or other assigned supervisory personnel to assure that the highest degree of quality compassionate resident care is maintained at all times. Care Partners are customer service experts who interact with leadership, families, vendors, guests, and other associates while displaying exceptional professionalism.

Essential Functions

1. Help create an atmosphere of compassion, optimism, warmth, resident interest and positive emphasis, as well as a calm environment throughout the Community
2. Cares for resident per the shift assignment determined by Charge Nurse or Health and Wellness Director.
3. Responsible for change of shift handoff report to following shift’s Care Partners.
4. Care Partners who are also certified Med Techs:
   1. May supervise self-administration of medication and treatments as prescribed by the health care provider; during the med pass, the Care Provider observes the resident’s response to the existing treatment plan, is aware of prominent medication interactions and side effects, and observes residents for the occurrence of such and notifies supervisor. The Care Partner, if working as a Med Tech, may communicate with pharmacies and or health care providers as necessary to ensure that residents in the Community have medications as ordered and must document accordingly.
   2. Assist with completion of medication cart audits per policy and direction of supervisor 
5. Document an accurate accounting of leisure activities, incidents and observations, and any changes in the physical or emotional condition of each resident
6. Maintain confidentiality and privacy of resident care, procedures, and documentation, and communicate with the resident and families on an adult and mature basis with dignity and respect for the resident’s rights and wishes
7. Report any resident abuse, accidents or incidents immediately to supervisor
8. Report changes in the resident’s condition to the charge/staff nurse as soon as possible
9. Report all complaints and grievances made by residents, families, visiting individuals, agencies, etc. as well as unauthorized trips the resident makes outside the Community to a supervisor
10. Assure that call system is within easy reach at all times and answered promptly 
11. Assist with caring for residents’ eyeglasses, hearing aids, prostheses, etc.; Assists in maintaining a physical, social, and psychological environment that is conducive to providing the best interest and welfare of the resident
12. Bathe, toilet and dress residents; Assist with Activities of Daily Living (ADL)
13. Follow established safety precautions and observe, monitor and intervene or report unsafe conditions in the
    Community.  I.e. checking and reporting lightbulbs when they need to be replaced, changes in room temperature, etc.
14. Review Resident Assessment and Plans of Care to residents’ conditions, etc. and explain each procedure to the resident as care is given
15. Follow established universal precautions and isolation procedures and report any communicable or infectious diseases that you may contract
16. Keep resident’s personal articles off of the floor, properly stored, as well as keeping an inventory and assisting to identify resident’s personal possessions on an on-going basis
17. Keep supply room, work areas, and equipment clean
18. Assist residents in preparing for and/or engaging in meals
19. Assist and participate with residents during activities, and social programs
20. Other duties as assigned by management

DISTINGUISHING CHARACTERISTICS:
This is the second in a series of five nursing-related job classifications within the Medical job family. This classification practices under the supervision of a Physician, Registered Nurse, or other similarly licensed supervisory health care practitioner and performs direct patient care functions within the scope of the vocational nurse licensure. Incumbents perform nursing duties of moderate complexity that require a moderate degree of initiative and independent judgment. This classification is distinguished from the Registered Nurse I and Registered Nurse II in that incumbents have not graduated from an accredited Registered Nursing program or do not coordinate the medical treatment of patients. Duties and Responsibilities

• Responds to patients' medical needs. Provides and/or arranges for treatment according to protocol and/or direction from authorized medical staff.
• Consults with clinical and administrative staff about patient health care issues.
• Responds to and assesses certain categories of emergent patient illnesses and injuries.
• Provides direct health care to patients, including wound care, medication administration, monitoring of vital signs, and laboratory procedures.
• Assists other medical staff, as required.
• Practices safety, environmental and/or infection control methods.
• Documents patient's condition and maintains patient medical files and records in computer. Performs other administrative duties.
• Inventories medications on a daily basis.
• Assists in dental, STD and other clinic settings.
• May answer medical questions from staff, patient's relatives and others as appropriate.
• May process patients into the facility by screening and evaluating their medical and mental histories.
• May assist patients with activities of daily living, including bathing and feeding.
• Performs other job-related duties as assigned.

Minimum Requirements
Education and Experience:
Graduation from an accredited Licensed Vocational Nursing program AND one (1) year of Licensed Vocational Nursing experience.

Licenses, Registrations, Certifications, or Special Requirements:
Current license to practice as a Licensed Vocational Nurse in the State of Texas.
Current Cardio-Pulmonary Resuscitation (CPR) Certification.

Work Environment & Other Information

• Assumes responsibility for the successful integration of professional staff.
• Plans and provides for department in-service presentations on a regular basis.
•           Assures completion of Performance Evaluations for staff in writing on a yearly basis.
• Coaching and teaching staff on an ongoing basis, for nursing skills development, providing feedback and disciplinary action when needed.
• Delegates duties as necessary to nursing management.
• Conducts regular nursing and other milieu meetings.
• Performs other duties in a willing manner or as requested by the CNO/DON, including assisting the Charge Nurse, Staff Nurses, LPNs, MHTs and other non-professional staff, as required.
